President’s Invite

Dear Colleagues,

It is my great pleasure to invite you to Boise for the 112th Annual Session of the Idaho State Dental Association. This year’s session will be held at the Boise Centre on the Grove, June 18-21, 2008.

We have an excellent line-up of speakers headed by Dr. John Kois, discussing “Interdisciplinary Treatment Planning I: Diagnostically Driven.” We also look forward to other outstanding speakers as well. The program will be of great interest to doctors and staff alike. In addition, you will see an expanded array of vendors.

There are several events planned including a golf tournament, President’s reception/silent auction, spouses’ event and, for the entire family, the Friday Fun Night with the “Rock & Soul Review” at the Old Idaho Penitentiary and white water rafting on the Main Payette on Saturday.

Boise, in the summer, is spectacular and we know you will enjoy the continuing education presentations, the exhibits, the social events, and all the outdoor recreation the area has to offer. Be sure to make your reservations now for staff and family and join us in “Believe It & Achieve It!”

Registration Deadlines

May 30 Early Bird Registration fee discounts deadline

May 30 Pre-Registration deadline
Receive registration materials by mail if registered by the deadline. If registration is received after this deadline, all registrants will pick up packets on site.

June 2 Refund/Cancellation request deadline
All requests for refunds must be made in writing to the ISDA office. Refunds less $10 processing fee will be given if the request is received by June 2, 2008.

Mail registration forms found in the back of this booklet together with your payment prior to May 30, 2008 and receive your registration packet by mail. You may also register online at www.isdaweb.com. BE SURE TO BRING YOUR PACKET WITH YOU TO THE MEETING!

Registration forms postmarked after May 30, 2008 will be considered on-site registration for fee and ticket purposes. Name badges, tickets and programs must be picked up at the registration table in the lobby of the Boise Centre on the Grove after 3:00 p.m. Wednesday, June 18, 2008.

Forgot your registration packet?
Those who forget their packet will have to go to the registration desk in the lobby of the conference center and have another one re-issued. There will be a $10 fee per registrant for processing duplicate registration materials.
